FRAMINGHAM – The MetroWest Health Foundation, in partnership with local health and human service organizations, is asking MetroWest residents to share their feedback on health issues and services in the region.

The 2023 Community Health Assessment survey is part of a multi-tiered process for capturing the many voices of the MetroWest region. The survey is short and
anonymous.

There are no right or wrong answers – it is your opinions and experiences that matter.

Information gathered from this survey will inform future health programming and community services in the MetroWest region. The completed Community Health
Assessment will provide an update from the last assessment conducted in 2019.

Past assessments have informed community efforts related to issues such as access to care, behavioral health and healthy aging. The end product of the 2023

Community Health Assessment survey will be made available in the fall at mwhealth.org.
