City to Host Two Public Distribution Days for Rapid COVID-19 Test Kits – Thursday, December 30, and
Saturday, January 8, 2022.

Registration Link Available on the front page of the City’s website and COVID-19 Page Thursday, December 23, (tomorrow) at 1 p.m.

To accommodate residents, the City will host two public distribution days, one during the week and one on the weekend.

Please remember the following when signing up for your test kit when the registration link is posted at 1
p.m., Thursday, December 23.

The Main Library, 49 Lexington Street, will have several designated
computers available for registration if needed.

o Priority for the public distribution will be given to Framingham residents ONLY who attest
to financial hardship. Once tests have been assigned, the online application form will be
closed.


o An email will be sent, including details for picking up test kit distribution (location).


o SUBMISSION OF THIS FORM DOES NOT GUARANTEE A TEST KIT!


o On the day of pick-up, bring and show a copy of the email verification (electronic or print)
and proof of residency (driver’s license, another form of identification, or piece of mail).
Names and addresses will be verified on the day of the pick-up.


o Only ONE form submission per household. Duplicate form submissions with the same IP
address WILL BE DELETED.

o There are two tests in each test kit. A maximum of two kits (four tests total) will be provided
per household.

o Residents will be able to choose from one of two collection days.


o Face coverings are REQUIRED when collecting test kits.


o If inclement weather occurs, the distribution will be rescheduled.


o Please do not call city departments to request a test kit. At this time, the City is NOT
keeping a phone-based list for public distribution.


o The City recognizes that the demand for test kits far exceeds the number the State has
provided. The City will make additional announcements regarding future distributions.
